Jewel-Osco Flu Shot Clinic at Concordia Church

The Health and Wellness Cabinet of Concordia has invited Jewel Osco to administer flu shots in the Fellowship Hall for your convenience. Immunizations will be available the morning of September 29^th to individuals 18 years of age or older. The shots will be available at the end of the 10:30 a.m. service.

Request forms for flu shots will be distributed prior to immunization at Concordia. Pre-registration is required to give the Jewel Osco pharmacy personnel time to process insurance information prior to the clinic date. There is no co-payment unless required by your insurance.
